An initiative funded by the National Alliance for Hispanic Health, "Vive Tu Vida! Get Up! Get Moving!" Provides community-based initiatives and activities designed to promote health and wellness among individuals. The primary goal of the program is to address fitness and healthy weight by utilizing local resources in the community such as cultural centers, parks and local soccer/sports teams to emphasize the importance of physical activity, supporting the family and community building.

The Alamosa Outreach Program is located within the City of Albuquerque's Alamosa Multi-Purpose Center located on Albuquerque's West Side. The program provides a variety of services including case management and structured recreational programs to deter youth from involvement in gangs, vandalism and serious crimes.

Casa Hermosa is an 18-month transitional living program for youth aged 16-21 that are runaway, homeless or aging out of state custody (foster care, institutional care). Casa Hermosa operates 24 hours a day, 7 days a week, 365 days a year in a staff-supervised apartment setting. The program offers the following services: crisis intervention, assessment, counseling/therapy, recreational activities, life and social skills training, case management, educational/vocational assistance, job training skills and referral assistance.

The Family and Community Services Program provides comprehensive services to residents of the South Valley and the unincorporated part of Bernalillo County. It offers afterschool sports and recreation, tutoring, arts and crafts, family support services, referral services, gang and substance abuse prevention, community training and empowerment activities. The program also offers services for senior citizens such as a commodity foods program, transportation and recreational/leisure activites.

The Gang Intervention Program works specifically with individual gang members and youth at risk of being involved with a gang in Albuquerque neighborhoods.  The program's goal is to stop the violence and keep YDl's communities safe. The program serves 350 youth annually and incorporates both prevention and intervention/diversion activities.

The Gang Intervention Program uses innovative approaches to help youth learn and experience positive alternatives. Services provided include case management, individual/family counseling with a licensed therapist, recreational activities and participation in community events. A key program activity is the Si Se Puede Leadership Program, an 8-week leadership series offered four (4) times a year for gang involved youth.

Project Achieve is a dropout prevention program for middle school students. The dropout prevention services include: classroom instruction in English, academic and career guidance, work experience, employability skills training, life skills education and post-secondary exploration. The services may include, but are not limited to, substance abuse prevention and rehabilitation, counseling and mentoring. The program also has a leadership development component that is designed to identify those at-risk students that have positive leadership potential. The intent is to develop that leadership through positive constructive activities.

The Family Therapy Project uses an effective, practical and problem focused model called Brief Strategic Family Therapy (BSFT). This clinical intervention model has been proven to successfully reduce problem behaviors in children and adolescents while strengthening the family. The program works with children/youth age 6-17 and their families. Brief Strategic Family Therapy is delivered in 8-12 weekly sessions of 1 to 1 1/2 hours each session. The project has an 84% success rate with families.
