Who We Are

Nurse-Family Partnership for Youth Development, Inc. is a trusted, community-driven nonprofit committed to improving the health, development, and economic stability of mothers and young children across Bernalillo, Torrance, Rio Arriba, Sandoval, and Valencia counties in New Mexico. As a proud implementing partner of the evidence-based Nurse-Family Partnership (NFP) model, we deliver expert, nurse-led home visitation services that begin in early pregnancy and continue through the child’s second birthday—supporting families every step of the way.

Every YDI- NFP Nurse is a licensed Registered Nurse with a Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN) degree with advanced clinical experience in the key areas, including:

  • Adult Health
  • Women’s Health
  • Pediatrics
  • Postpartum Care
  • Labor and Delivery

What We Offer

Our program pairs families with a dedicated nurse who provide ongoing, in-home visits focused on:

  • Healthy pregnancies and positive birth outcomes
  • Infant growth, safety, nutrition, and early development
  • Maternal mental health and wellness
  • Parenting education and support
  • Family goal setting, education, and employment pathways
  • Connection to healthcare, and community services

Who Is Eligible?


Our services are available to pregnant individuals who:

  • Are eligible or enrolled in Medicaid
  • Are pregnant (ideally early pregnancy but exceptions can be made)
  • Live in Bernalillo, Torrance, Rio Arriba, Sandoval, or Valencia County

Proven Results

The Nurse Family Partnership model is a nationally recognized model and backed by decades of research. A 2020 study published in Public Health Nursing ¹confirms that mothers enrolled in Nurse Family Partnership programs are more likely to complete high school/GED programs, gain employment, and establish long-term family stability, benefiting entire communities.

The New Mexico Early Childhood Education & Care Department aims to is to create a more cohesive, equitable, and effective early childhood system in New Mexico. That means coordinating a continuum of programs from prenatal to five—and ensuring that families in every corner of the state can access the services they need.
